LICENSED PRACTICAL NURSE JOB DESCRIPTION
JOB SUMMARY
The Licensed Practical Nurse (LPN) responsibilities include providing for the emotional, medical, and physical needs of Danville’s clients and improving their quality of life by observing, documenting, reporting, taking action and following-up on their health, safety, and medical needs, and implementing medical recommendations made by the team. Per state and Nurse Practice Act requirements, LPN’s provide nursing care under the supervision of a Registered Nurse (RN).
ESSENTIAL JOB FUNCTIONS
• Behavior should be professional at all times and consistent with Danville’s culture, mission, values, philosophy, and policies and procedures, including treating clients and others with dignity and respect.
• Assess, document, and report to the appropriate management any changes in clients’ condition, including signs and symptoms which may be indicative of changes in their overall health.
• Support and advise employees and management in supporting clients in self-administering medication and topical treatments as prescribed and documenting according to Danville’s policies and state protocols.
• Ensure clients’ medical and dental needs are met, including, scheduling and ensuring clients attend doctor or dental appointment, keeping immunizations current, informing management of emergency room visits and/or hospitalizations, following through with physicians’ orders and recommendations
• Ensure clients’ privacy is respected at all times in compliance with HIPAA privacy regulations.
• Act as a resource to direct support employees, including conducting health service orientations, providing training to employees to ensure they can competently medically assist clients.
• Report to work on time, and if unable to report to work, or if there is a need to come to work late, or leave early, follow Danville’s approval policy.
• Complete required pre-service and on-going training requirements within Danville and state required timeframes and meeting region required competency levels
ADDITIONAL JOB RESPONSIBILITIES
• After being properly trained, perform additional nursing responsibilities per clients’ individual needs and Health Care Provider’s orders.
MINIMUM QUALIFICATIONS
Education and Experience
• LPN Degree from an accredited school of nursing
Additional Requirements
• Required to pass a criminal background check upon hire, and annually
• Must be 18 years or older
• If responsible for driving on company business, must be 21 years of age or older, have a valid state driver’s license, proof of automobile insurance, and an insurable driving record
• Food Handlers and/or TB tests, per specific state requirements
Knowledge Skills and Abilities
• Ability to lift 50 pounds
• Basic fluency in oral and written English
• Knowledge of Microsoft Office
• Must be highly organized
• Must have good interpersonal communication skills
Special Skills/Training/Certifications
• Must have an unencumbered state LPN license
• CPR/First Aid
